Not all open worlds have real time elements. Not all open worlds take place outside. No all open worlds are to scale with in game characters or lore. "Large" isn't even a good definition as it all depends on what you mean by large. Skyrim is tiny compared to Just Cause 3, which is small compared to Minecraft, and that is small compared to No Man's Sky.
